Google Forms can do magic.

With Google Forms you can collect, distribute, analyze, and share data effortlessly. Send out a link or embed your form directly in an email or web page. Really liked your article - Simplify Data Collection with Google Forms. I used Google Docs to help arrange a high school reunion and although I didn't use as many of the features as your video describes, I found it to be a very useful, time saving tool.
