From the GMR: Community Transformation Grants (CTGs) and May 25th Teleconference
The Centers for Disease Control and Prevention (CDC) recently announced the availability of $100 million to support Community Transformation Grants. A conference call prior to the June 6th deadline for submitting Letters of Intent to apply for funding will be held on Wednesday, May
Community Transformation Grants (CTGs) are authorized under The Patient Protection and Affordable Care Act (ACA) of 2010 for state and local governmental agencies, tribes and territories, and national and community-based organizations.
The CTGs will support the implementation, evaluation, and dissemination of evidence-based community preventive health activities to reduce chronic disease rates, prevent the development of secondary conditions, address health disparities, and develop a stronger evidence base for effective prevention programming.
Funding is available to support evidence and practice-based community and clinical prevention and wellness strategies that will lead to specific, measurable health outcomes to reduce chronic disease rates.
Additionally, these grants were a key component of the Prevention Fund supported by the American Public Health Association (APHA) and other public health advocates for inclusion in the ACA. The CDC will be holding a conference call prior to the deadline for submitting Letters of Intent to apply for funding. The Letters of Intent are due to CDC on Monday, June 6, 2011.
