Survey Instruments

The SPLASH survey will be conducted during the fall of 2010 and 2011. Researchers will ask questions on individual demographic background, weekly park use, leisure activity patterns, and general health status. Roughly 600 households will be recruited each year to participate in the SPLASH project.

Survey instruments include:

  • SPLASH Community Survey

  • 3 Day Park Use Recall

  • SF 12 Adult Health Questionnaire

  • SF 10 Child Health Questionnaire
